Depending on which area of the hospital you are seen in, within 7-10 days of your appointment (or discharge) you will receive one of the following:

• text message survey – you will receive a text message from #91994
• survey in the mail – you will receive a survey in the mail to your home address
• email survey – you will receive an email from Southwest Health
