If you are recovering from a heart attack or heart surgery, you’ll need to make some lifestyle changes, though you may not be sure how. Our cardiac rehabilitation program can help.

Cardiac Rehab at SHC gives you the tools to take charge of your heart health—to get well and to prevent another attack.

Our supervised two-phase program offers a personalized exercise routine, education about medications and nutrition, and emotional support so you can resume normal activity. Our rehab Phases II and III both take place after your Phase I hospitalization.

Our Program

Cardiac Rehab Phase II

Phase II is an outpatient rehab program designed to reinforce what you learned during Phase I hospitalization. Rehab begins one to six weeks after hospital discharge. Sessions are held three times per week with session length determined by your specific needs.

Cardiac Rehab Phase II may be covered by private insurance, Medicare, or self-pay. Coverage is determined prior to or at the time of admission to the program. Your physician will initiate a referral for your participation in Cardiac Rehab. We also accept referrals from other hospitals/facilities.

Cardiac Rehab Phase III

Phase III rehab is a self-pay outpatient service designed to follow your completion of Phase II. The primary goals are to improve physical fitness, promote a feeling of wellbeing and reduce risk of a recurring event.
